Eligibility:

Eligibility & Sportsmanship/Ejection Requirements and Guidelines

Required Forms:

All ACS students who interested in trying out for a sports team MUST submit the following paperwork:

1. Student-Athlete Participation Packet;

2. Athletics Student Pick Up Liability Form;

3. NCHSAA Medical Exam Form – Participation Physical Exam is required for participation

        There are two: PPE English and PPE Spanish;

4. Gfeller-Waller NCHSAA Student-Athlete & Parent/Legal Custodian Concussion Statement Form.
